Hidden Costs of a Hair Transplant: Meds, Touch-Ups, Time

The hidden costs of a hair transplant are the lines that sit outside the headline price: tests before surgery, medication, follow-up visits, a touch-up or second session, time away from work, and interest if you borrow. Some clinics include several of these; others bill each one. None is secret, but each is easy to miss when you are comparing totals. Here is the list, with what we could verify about each on 22 September 2026.

Why the headline price is not the bill

A quote is built around the surgery day, and the day is only part of the cost. The published US guides we read put FUE between $4 and $12 a graft, which already makes the operation expensive, so the extra lines are easy to treat as small change. Over a few years they are not. The fix is simple: ask every clinic for the same list, in writing, and add the lines yourself.

Pre-operative tests and medical clearance

Many surgeons want blood tests before operating. In the ISHRS 2025 Practice Census, 55.3% of responding members said they require pre-operative labs and 25.1% said they sometimes do; the tests they named most were a complete blood count, HIV and hepatitis. For older patients, 66.1% of members said they require medical clearance, at a mean age of 60.

Ask whether these tests are in the quote, done by the clinic, or ordered through your own doctor, where insurance may cover them. A test done twice because nobody checked is a cost you can avoid.

The consultation itself

Some consultations are free and some carry a fee, sometimes credited against surgery if you go ahead. HairClub, for example, describes its consultation as complimentary. Ask before you book whether there is a fee, whether it is refundable, and who you will see: a surgeon, a nurse or a sales consultant. A consultation with the person who will operate is worth more than a free one with someone who will not.

Medication, possibly for years

This is the line that is easiest to leave off. The American Academy of Dermatology notes that medicine can prevent or slow new hair loss, which is how the native hair around transplanted grafts is protected. Transplanted follicles are usually resistant to the hormone behind pattern hair loss; the hair around them is not.

We do not print medication prices here. Ask your doctor what they would prescribe, ask a pharmacy the price of the generic, and multiply by the years you expect to take it. Generic minoxidil from any pharmacy and generic finasteride on a prescription filled locally are the usual options to discuss with a doctor, and neither pays us anything.

Post-operative supplies and aftercare

Clinics differ on what they send you home with: washes, sprays, dressings, pain relief, a pillow or a hat. Some include a kit; some sell one. It is a small line, but it tells you something about how the clinic prices, and it should be on the written quote.

Aftercare instructions also cost time, since the first days usually mean careful washing and sleeping positions. Ask how many days the clinic expects you to need at home.

Follow-up visits

A good clinic checks healing and growth over months. Ask how many follow-up visits are included, over what period, and what a visit costs if you need one outside that window. If you are travelling to the clinic, each follow-up has a travel cost too. The AAD says most patients see results six to nine months after surgery, and some wait 12, so follow-ups naturally stretch across most of a year.

Add-on treatments

Some clinics offer extra treatments alongside surgery and price them separately. If one is offered, ask for it as its own line with its own price and ask what evidence supports it in your case. An add-on bundled into a package is impossible to compare; the same add-on priced on its own line is easy. You can always decline it and ask for a revised quote.

Touch-ups and second sessions

Cleveland Clinic notes that some people need several touch-up procedures for a natural-looking result. The census puts the mean number of procedures per patient at 1.5, with a mean of 1,637 grafts for later sessions. Ask the clinic in writing whether a touch-up is included, discounted or priced like a new operation, and within what period. Our guide to second-session and repair costs goes further.

Time off, travel and the people around you

Time off is a real cost even if your employer pays sick leave, and it is unpaid for many self-employed people. The AAD says the surgery itself takes four to eight hours, and larger sessions may need you back the next day. Add travel, parking or a hotel if the clinic is far away, and possibly someone to drive you home. None of this appears on the quote unless you add it.

Interest, if you borrow

Borrowing turns one price into a larger one. A fixed-rate loan adds interest you can calculate in advance. A deferred-interest medical card adds nothing if you clear it in time and a great deal if you do not: the Consumer Financial Protection Bureau found patients paid $1 billion in deferred interest from 2018 to 2020. Build a simple table with one row per line above and one column per clinic. Fill in what each quote includes, ask for prices on the rest, and add the column. The total at the bottom is the number to compare, not the one on the front of the quote. Costs that change with the clinic's location

Where the clinic is changes some lines and not others. A clinic in another city adds travel and possibly a hotel for the surgery, and again for each follow-up you attend in person. Some clinics offer remote follow-ups by photo or video; ask whether that is an option and whether it is included.

Location also affects what happens if something goes wrong. A problem found weeks later is easier and cheaper to deal with when the surgeon is within reach. That is not a reason to choose the nearest clinic, but it is a reason to ask any distant clinic how it handles complications after you have gone home, and who pays for that care.

The cost of not asking

A repair is among the most expensive hidden costs. ISHRS members told their society that repairs of illicit work made up an average of 10% of their cases. Paying a little more for a clinic that answers every question in writing is cheaper than paying twice.

Sometimes a starter supply is, but ongoing medication almost never is. Ask each clinic what it includes, then price anything longer term at a pharmacy.
Do I have to pay for follow-up appointments?
It depends on the clinic. Many include a set number of follow-ups within a period; ask how many, over how long, and what extra visits cost.
Is a touch-up included?
Some clinics include or discount one within a set time; others price it as a new procedure. Get the clinic's policy in writing before you pay a deposit.
